Image Consent

I hereby consent to the use of my name, voice, recording, photograph, likeness, or reproduction, as well as the likeness or image of any pets present, by videotape and/or film for commercial and promotional use by PINK CAMERA PRODUCTIONS and/or EAST LAKE LP d/b/a ABBY’S DOG DAYCARE at any time in the future.

I release PINK CAMERA PRODUCTIONS and/or EAST LAKE LP d/b/a ABBY’S DOG DAYCARE from any and all liability arising from the use of aforementioned material in print, on social media, on electronic communications including websites and/or in any other media outlets.

I understand that no compensation will be due to me, and that in giving this consent I hereby waive any right to approve accompanying written or narrative material.

I represent that I am at least eighteen (18) years of age.
